For each visitor to our Web page, our Web server automatically recognizes no information regarding the domain or e-mail address. We collect the e-mail address of those who communicate with us via e-mail, aggregate information on what pages consumers access or visit, user specific information on what pages consumers access or visit and information volunteered by the consumer, such as survey information and/or site registrations.

The information we collect is used for internal review and is then stored in a locked facility until you need it back or let us know we can destroy it by your case being closed, It is only used to help with your case and your case alone. It will never be shared without your consent.

If you do not want to receive e-mail from us in the future, please let us know by sending an e-mail, calling or writing, and telling us that you do not want to receive e-mail from our company.

NOTICE OF PRIVACY PRACTICES
Maintaining your trust and confidence is a top priority to Slade Family Services. This notice lists your rights under my privacy policy. You have the right to your protected information, the right to amend your protected information, the right to an accounting of disclosures and the right to request restrictions on your information that you give to Slade Family Services.

I. I have a legal duty to safe guard your protected information (PI).

I am legally required to protect the privacy of your PI, which includes information that can be used to identify you that I have created or received about your past, present, or future. I must provide you with this notice of my privacy policy and practices, and such notice must explain how, when, and why I will “use” and “disclose” your PI. A “use” of PI occurs when I share, examine, utilize, apply, or analyze such information within my office. PI is “disclosed” when it is released, transferred, or otherwise divulged to a third party outside of my office. With some exceptions, I may not use or disclose any more of your PI than is necessary to accomplish the purpose for which the use or disclosure is made, and I am legally required to follow the privacy practices described in the notice.

However, I reserve the right to change the terms of this Notice at any time and my privacy policy at any time. Any changes will apply to PI on file with me already. Before I make any important changes to my policies, I will promptly change this notice and post a new copy of it in my office and on my website. You can request a copy from me or you can view it in my office or on my website.

II. How I may use and disclose your PI.

I will use and disclose your PI for many different reasons. For some of these uses or disclosers, I will need your prior authorization and for others I do not need your authorization. I may disclose your PI to others without your consent in certain situation. For example, your consent is not required when it is requested by federal, state, or local law enforcement or judicial or administrative proceedings. I need your consent to disclose information to family friends or others. I will ask for your written authorization before disclosing any of your information to family friends or others.

III. Your Rights.

You have the right to request limits on uses and disclosures of your PI. You may not put a limit on disclosures I may be legally required to use. You have the right to choose how I send you your PI. You have the right to ask that I send your information to an alternative address or by alternative means. To your work or to your home, by regular mail or by e-mail. You have the right to see and get copies of the PI that I have, but you must make this request in writing. You have the right to request the disclosures that I have made and to whom. You also have the right to update or correct your PI.
